We empower individuals and their families by providing information and resources to make informed decisions and actively participate in the planning and decision-making process.
Monitoring and Review: Our support coordinators regularly review the individual's plan to ensure it remains relevant and aligned with their changing needs and goals. We monitor the delivery of services, assess outcomes, and make adjustments as necessary to optimise the support provided.
Collaboration and Network Building: Our support coordinators collaborate with a range of service providers and community organisations to ensure a holistic and coordinated approach to the individual's support.
