Our support coordinators work closely with individuals and their families to develop and implement a coordinated plan to address their specific needs and goals. We assist in navigating and accessing the supports and services available to them.

Support Coordination at Real Care Given can include:

  1. Service Coordination: Support coordinators facilitate the coordination and arrangement of services and supports outlined in the individual's plan.   3. Advocacy and Empowerment: Our support coordinators advocate for the rights and needs of individuals with disabilities, ensuring their voices are heard and respected. We empower individuals and their families by providing information and resources to make informed decisions and actively participate in the planning and decision-making process.

  4. Monitoring and Review: Our support coordinators regularly review the individual's plan to ensure it remains relevant and aligned with their changing needs and goals. We monitor the delivery of services, assess outcomes, and make adjustments as necessary to optimise the support provided.

Collaboration and Network Building: Our support coordinators collaborate with a range of service providers and community organisations to ensure a holistic and coordinated approach to the individual's support. We build and maintain networks to facilitate effective service delivery and enhance the individual's access to resources.
